Didn't Watanuki's parents' die in a car crash? Perhaps his parents/whoever was driving was drinking prior to the accident. He loves his parents but knowing that alcohol killed them must've been particularly harsh.
Like Mokona remarked, his heart - or at the very least, his head - may not remember his parents' names and much else, but his body (and subconscious) will remember how it felt to lose parents to an alcoholic related incident.
Maybe Watanuki is just against seeing the people he is really close to drinking alcohol because it took away those especially close to him. At some point, I'm sure he has seen Himawari drink and he has also seen the oba-chan fortune teller drink as well. But he doesn't seem to be as adverse to them drinking as he is with seeing Yuuko and Doumeki.
Maybe Watanuki is suffering from a form of social phobia. Again, he lost his parents who were close to him, I presume. If he feels, even subconsciously, that it is possible to become close to someone in as deep of a manner as with his parents, he'd react violently (with a cat-like freak out according to Himawari's imaginations, LOL). Sure, he is close to Himawari, but not as close as he is with Doumeki and Yuuko, two people that he has freaked out on plenty of times.
